The purpose of the 2 thumb loops is for different hand sizes.
Larger hands should use the larger thumb loop, while smaller hands will find the smaller thumb loop more comfortable.
We have seen people using the 2nd loop on the index finger / middle finger, which is also an option, but it was not originally designed to use both loops at the same time.
